We want you to have a full understanding of the financial policies of the office.

Financial Responsibilities:

• In order to maintain a successful relationship, good communication between you and the office is very important. In this regard, we expect you to bring your insurance card to every visit.
• Our staff is very knowledgeable about insurance, and can answer many questions, but it is your insurance company who makes the final determination about eligibility and benefits.
• We participate in most healthcare plans, including Medicare. Please contact your insurance carrier prior to your visit to confirm our participation with your specific plan.
• If your insurance requires a referral from your primary care provider, that referral must be obtained prior to your visit.
• You are responsible for any copays, deductibles, and coinsurance as dictated by your plan at the time of your visit.
• We accept cash, checks, most credit cards, and contactless payments (Apple Pay and Google Pay).
